Aging and Disability Resources for Colorado

Empowering older adults, adults with disabilities, and caregivers to navigate health and long term care options.

The Aging and Disability Resources for Colorado provides options counseling for seniors or adults with disabilities and their caregivers to help provide guidance and insight on what programs and long term support services would best fit that individual’s needs. Option Counseling can be done over the phone, by appointment, or by home visit with one of ADRCs options counselors.

Home Connections

The goal of Home Connections is to help seniors remain safe and independent in their home by providing homemaker services, electronic monitoring, and case management including accessing services, home visits, and transitioning to higher levels of care..

Veteran’s Services

Veterans Directed Home and Community Based Services is a program for Veterans of any age who are determined by the VA to be at risk. With the help of a case manager, Veterans can direct their own care by managing flexible spending budget and personal care providers.

Supporting Our Seniors (SOS)

SOS is a transportation service for homebound seniors 60 and over. SOS provides seniors with one ride a week to doctor/ medical appointments or pharmacy pickups. This program is free and volunteer based. Apply here to get the process started.
